For measuring retinal acuity, the preferred method is the preferential looking technique, which is described in detail in U.S. Pat. No. 3,907,807, to L. B. Howard, Jr. entitled, “Perceptually Adaptive Acuity Test.” One of the problems associated with the preferred technique is that it is only capable of measuring acuity with a limited range of contrast levels. For example, a testing range of 0.0 to 5.0 (log contrast units) is considered adequate.
The visual acuity of an infant can be as low as 20/60, and there are no accepted techniques or even standards for testing infant visual acuity. It is usually determined by the practitioner by trial and error. For this reason, significant variability has been found in the early evaluation of babies with retinopathy of prematurity. It is also very difficult to distinguish a refractive error from a retinal abnormality, because the parents do not know the reason for any loss of sight in their child, and therefore have no frame of reference for comparison.
It is known that the contrast level needed to produce a particular level of acuity varies with retinal location within the eye, i.e., around a fixation point. Since the contrast level required to determine acuity at a given point varies with location, different contrast levels must be used to test retinal acuity at different locations. When the same contrast level is used throughout the test, acuity is measured only at the same location. Because of this, acuity at a test location is never uniform, and generally it is lower than acuity at all other retinal locations. The uncertainty of the test occurs both between locations and between patients. For example, a patient with an acuity of 20/30 in the right eye and 20/20 in the left eye has a uniform acuity of 20/30 throughout. But, a patient with a 20/60 acuity in one eye and a 40/60 acuity in the other eye might achieve an acuity of 20/80 by testing all retinal locations, but might achieve 20/20 by testing only the most peripheral location.
